The game is set in an alternate future that branches off from our reality starting in the late '80s. The narrative revolves around the emergence of a new dominant religion within the Intergalactic universe. Naughty Dog dedicated several years to crafting the rich lore of this religion, tracing its journey from the inception by its first prophet through its various transformations and eventual distortions.

This new religion originates and spreads across a single planet, which over time becomes isolated from the rest of the galaxy. The protagonist of the game crash-lands on this very planet, finding herself in a desolate place where she is completely alone. Survival becomes the central theme as she navigates this solitary existence. Druckmann highlighted that, unlike previous Naughty Dog titles where players often had a companion for guidance, in Intergalactic: The Heretic Prophet, players will have to solve challenges on their own to find a way off the planet.
