Despite no formal statement from developer Konami, the game has been rated Mature 17+ by the U.S. classification board due to its intense elements such as realistic gunfire, cries of pain, bloody combat, and "suggestive/sexual content."

The ESRB's detailed report on the game emphasizes its realistic portrayal of violence and bloodshed, citing examples such as cutscenes showing a restrained character being beaten and electrocuted, a character being shot in the eye, and another set on fire and shot multiple times. Additionally, the ESRB notes the presence of "suggestive/sexual content," which includes scenes of a man groping a woman's breasts, close-up camera angles focusing on deep cleavage, a character briefly groping a man's crotch, and the Peep Demo Theatre feature. This feature, found in the Subsistence and HD Collection versions of the original Metal Gear Solid 3, allows players to view cutscenes of a female character in her underwear from a first-person perspective after completing the game four times.
